Overview:
Warren Achievement Center is seeking a dedicated Case Management Administrator to join our team. As a Case Management Administrator, you will play a crucial role in supporting individuals with developmental disabilities to achieve their goals and lead fulfilling lives within the community. This position offers a rewarding opportunity to make a positive impact and contribute to the well-being of others.
Duties:
-Provides case management for adults enrolled in WAC programs.
-Performs the duties of Qualified Intellectual Disabilities Professional, as defined in CILA and other standards.
-Conducts Individual Service Plan staffing and special staffing.
-Convenes and conducts team meetings.
-Provides counseling services to individuals and their families.
-Coordinates transition services and provides provider administrator duties for Therap Services.
-Along with service planning and facilitation, monitoring, needs assessment, advocacy, discharge planning, follow up, and more.
Requirements:
– Bachelor of Science Degree in Social Work or Human Services
– Five years work or life experience with individuals with developmental disabilities.
– Strong organizational skills and attention to detail for effective case management.
– Excellent communication skills for building rapport with clients and collaborating with team members.
– Ability to prioritize tasks, problem-solve, and adapt to changing situations.
– Not required but would be nice to have QIDP certification through DHS.
– Previous experience in program development and implementation is desirable.

As a Caregiver/Direct Support Professional – DSP – you will earn a higher starting wage and a sign-on bonus with increases for training & shift differentials! Second & third shifts needed.
Do you have the skills to be a DSP / Caregiver?
Qualified applicants must have the following:
· Desire, compassion and ability to assist, support, and interact with individuals with intellectual disabilities
· High school diploma or equivalent
· Valid driver’s license with a good driving record
· Must be able to communicate in English
· Must be able to pass a background check
· Eligible to work in the United States
· Ability to use a desktop computer effectively for documentation and recordkeeping
Caregiver/Direct Support Professional Responsibilities:
· Caregiver to provide care, support, and supervision to individuals with disabilities in a group home setting.
· Support individuals with self-care, social and independent living skills, recreation and leisure activities.
· Perform household tasks, including meal preparation, laundry, and cleaning.
· Encourage independence, up to a person’s abilities.
The Caregiver/DSP, Direct Support Professional position can be any shift, some days during the week, and 8-hour shifts every other weekend. Several full time and part time positions are available.
